Documents you will need to complete your enrollment:
To complete the registration process at any of the Sunnyside Schools you will need the following for each student:
- Certified Birth Certificate
- Immunization Records
- Parent/Guardian ID Card
- Proof of Residence – 1 item -(i.e. Driver’s License/ID with Current Address, Rental Lease Agreement or Deed, Current Utility Bill)
- You will need to submit a separate enrollment form for each student you wish to enroll
Please Note: Parent or Legal Guardian’s Signature is necessary on enrollment forms.

If you do not live within SUSD boundaries OR if you are requesting to attend a different school than your boundary school YOU MUST SUBMIT THIS FORM IN ADDITION TO COMPLETING THE ENROLLMENT FORM.

The McKinney-Vento Homeless Assistance Act (McKinney-Vento Act) (42 U. S. C. § 11431-11435) is federal legislation that provides protections for children and youth experiencing homelessness, including enrollment rights. This legislation ensures that homeless children and youth have full and equal access to an appropriate public education and that they experience success in school.
