Permission to Evaluate

If an evaluation seems necessary, you will be asked to sign a Permission to Evaluate form. Various components of testing and observation will occur, and the results will be shared with you when the evaluation is completed. You will be invited to a meeting to discuss the results. This meeting is called a staffing, or a Multi-Disciplinary Conference (MDC).
